Installed 11.4 on my ipad and iphone and 4.3.1 on the apple watch. Messages now sync perfectly between the phone and ipad but not the watch. Deleted messages on the phone and ipad remain on the watch. Is this normal? Is there a watch setting that needs changed?

For some strange reason, I don’t believe messages are unified with the watch.
This is probably the most frustrating “feature” the Apple Watch has.

On my Apple Watch, every message sent to my iPhone is also received on my watch. I can open the messages app on my Apple Watch and read the message history, which is synced with my iOS devices and Macs. Open the Watch app on your iPhone and scroll down to Messages. Make sure you've selected the Mirror my iPhone option. Once you've done this The messages on your Apple Watch should be synced with your iPhone and other Apple devices. Check the Messages app on your Apple Watch to make sure. Messages sent since syncing started will be available.

If you delete a message on your phone, does it delete on your watch?

No. If I unpaired and re-paired my Apple Watch it would likely not have the messages deleted from my iPhone.

Okay. Sounds like mine is working properly. The messages are received on my watch but do not delete from the watch when deleted from the phone or iPad. I hoped I would not have to delete separately

But any changes you make in Messages on the Apple Watch, such as adding emphasis to an already received message are reflected in Messages on your other devices.

One of the new features of iOS 12 is cloud sync for Messages. Once it’s out, and you activate it, then messages will stay synced accross all your devices.

Untill then your messages to not really sync. What happens is that all your devices recieve the messages; but what you do with them on that device stays on that device.

Note: The iPad only gets all your messages (including SMS) if you turn on Text Message Forwarding in the iPhone’s Messages settings.

You’re right. I must be going senile. I could have sworn they talked about it in the last Keynote; but there is no mention of it when I look back through the WWDC 2018 articles. Maybe the Voice Memo syncing caused some kind of mental flash back.
