All I use Apple’s Reminders app, and I rarely do anything on the watch except receive the notifications. Lately I’ve been using the new Siri face, as it does a pretty good job of keeping events and notifications visible. On rare occations I have added a reminder using Siri on the watch; but prefer to pull my phone out of my pocket and use it instead. I usually have to go back and edit the reminder anyway, so it’s actually less work using the phone.
My ToDo needs are simple, so Reminders is all I need. That plus the better system integration keeps me from getting to excited about the many more powerful To-Do apps available.
However, I do read blogs from people who like or need more. These three apps are often mentioned, so they might be worth your consideration.
Todoist - Powerful, flexible, and cross platform
Things - Powerful, flexible, MacOS/iOS only.
OmniFocuse - profestional level to-do and project management.
I’m pretty sure all of them have watch components; but I could be wrong.
I like Swipes, which is a great little to-do app with a great Apple Watch app, too.
